• Class Fee:  $160.00 per Semester
  
  
• Number of Classes:  16 per Semester
  
  
• Class Location:  Recital Halls at Riverton Music in Sandy, UT.
  
  

• COST OPTIONS

1____  $10.00 per 45 minute lesson payment for both semesters $320.00
2____  $10.00 per 45 minute lesson payment for each semester $160.00 + $160.00 = $320.00
                                   
If a student withdraws:
     within the first two weeks of lessons, 80% of tuition is refunded.
    within the third and fourth week, 60% of tuition is refunded.
    within the fifth and sixth week, 40% of tuition is refunded.

Please complete and return this form and payment to the music school.  Financial commitment is per semester
(16 week/semester; two semester/school year).  Tuition will be prorated if lessons start after the semester has begun.
